2009-10 Washington Capitals: The winners of the Presidents' Trophy and No. 1 seeds in the Eastern Conference, they went up in the first round against the Montreal Canadiens and went up 3-1 in the best-of-seven series before blowing it.
2017-18 Virginia Cavaliers: Virginia was the top overall seed in the 2018 NCAA tournament. They became the first No. 1 seed to lose to a No. 16 seed in the history of the event. They lost 74-54.
